Well, make a simple menu with big buttons +, -, * and / and put their descriptions with it. When you move over stuff, something happens (for example lighting up).
You could use the KnP snail math game library as inspiration too.

What age range are we looking for here?
Usually younger kids would enjoy learning something with bright colors and shapes, while someone a little older would have adding and subtracting math. Then a little higher then that, you could start with multiplying and dividing. As far as the overall interface, bright colors, large objects, clean though, clutter is never good. Avoid gradients and solid colors obviously. Like DaVince said though, making things like lighting up, or maybe a small firework effect when you roll over clickable items, to make it easier for kids to tell what is and isn’t clickable.

Put some lasers in it. That always keeps me entertained.

Make it fun, put the math sneaky in the background so that kids think its more about the game then the math but when its really teaching them math

You could have some kind of rewardsystem, like when you have a certain amount of correct answers you get a medal or something...

You could make it so that you have to answer problems, rack up allot of points, then pay with those points, to play fun (and I mean fun... not pong or space invader remakes) games. Like... to get an extra life, you can pay with points earned.
