I can't find any way of enabling/disabling gamepad controls, or allowing users to customise the button configuration inside MMF2. Am I missing something?

I've only found the option to change default controls, which isn't what I'm looking for.

You can use the ControlX object or the Joystick2 Object. It's really simple.

Ah, are these extensions? I've been trying to avoid them, but may give in.

This article by Fifth is a very nice solution: http://create-games.com/article.asp?id=1677
It's for MMF1.5 so it's slightly out of date (there is probably a way to include Xbox 360 joypads in the mix by now) but it still works and is IMO the best article written about it.
