 Yes, I know it's a fangame (a Sonic fangame at that), and only a demo. But look at it - I think that the graphics and presentation set this apart from typical efforts at fangames. It features effects such as motion blur and the ever more familiar parallax scrolling, and there's even an online Time Attack mode. At 8MB it's a bit of a download for me on 56k, though... It was made by Jeff Y and his team, and is named Emerald Ties: Crossing Fates.

The plot behind Emerald Ties: Crossing Fates (ET:CF for short) is that the Chaos Emeralds have lost their power ans Sonic has to collect them all and find a way to restore the power. Sound cheesy and unoriginal? Well, the full plot actually does have some twists and stuff in it, but seeing as the demo only encompasses one level, you don't get much of a the story from it anyway.
